ABSTRACT

@#AIM: To investigate the changes and clinical significance of corneal parameters and tear film stability after cataract combined with pterygium surgery. <p>METHODS: The clinical data of 80 patients with cataract and pterygium admitted to our hospital from January to December 2020 were retrospectively studied. All patients underwent pterygium excision + phacoemulsification + intraocular lens implantation + limbal stem cell transplantation. The changes of visual acuity, corneal Pentacam parameters, corneal astigmatism, corneal curvature and tear film stability before and after operation were compared, and the degree deviation of intraocular lens(IOL)was calculated.<p>RESULTS: At 3mo after operation, the average corneal refractive index and average corneal refractive index of the patients in this group were significantly higher than those before operation. The index of surface variance(ISV)of corneal surface and astigmatism of anterior corneal surface were significantly lower than those before operation, and the indexes of tear film stability were significantly improved compared with those before operation(all <i>P</i><0.01). At 6mo after operation, the visual acuity of the patients in this group was significantly improved compared with that before operation, and the corneal astigmatism was significantly decreased compared with that before operation, but the corneal curvature was significantly higher than that before operation(all <i>P</i><0.001). The IOL degree deviation of the patients in this group was -5.5 - +3.5D.<p>CONCLUSION: Cataract combined with pterygium surgery can effectively improve the visual acuity of patients and improve the stability of tear film, but there may be corneal astigmatism. Therefore, the contralateral corneal refractive power should be used to calculate the IOL degree after pterygium excision. When the corneal refractive state is stable, cataract phacoemulsification is performed to ensure its prognosis.

ABSTRACT

@#AIM: To investigate the changes of corneal endothelial cells and ocular surface after phacoemulsification for age-related cataract(ARC)patients with diabetes mellitus. <p>METHODS: Retrospective case study. A total of 190 cataract patients with diabetes mellitus who received phacoemulsification combined with IOL implantation in 190 eyes admitted to our hospital from January 2017 to January 2019 were selected. In addition, 230 ARC patients without diabetes who underwent phacoemulsification and intraocular lens implantation at the same time were selected as the control group. Ocular surface disease index(OSDI)score, tear film rupture time(BUT), basal tear secretion test(S I t), corneal endothelial cell density and coefficient of variation were compared between the two groups.<p>RESULTS: There was no significant difference in preoperative OSDI score, BUT, S I t, corneal endothelial cell density and coefficient of variation between the two groups(<i>P</i>>0.05). In the observation group, OSDI scores were significantly increased 1wk, 1mo and 3mo after surgery compared with those before surgery, with statistically significant differences(all <i>P</i><0.01). The OSDI scores of patients in the control group increased significantly one week and one month after the operation compared with those before the operation, with statistically significant differences(all <i>P</i><0.01). The OSDI scores in the observation group at each time point after the operation were higher than those in the control group, with statistically significant differences(all <i>P</i><0.05). One week after surgery, 1mo after surgery, BUT and S I t in the two groups decreased significantly compared with that before surgery(<i>P</i><0.05), and the difference was not statistically significant compared with that before surgery 3mo after surgery. In addition, compared with BUT between the two groups, the observation group had a lower tear film stabilization time and a more significant decrease(<i>P</i><0.05). The corneal endothelial cell density in the two groups decreased significantly 1wk, 1mo and 3mo after surgery compared with that before surgery(<i>P</i><0.05). The variation coefficient of corneal endothelial cells in the two groups was statistically significant 1wk after the operation and 1mo after the operation compared with that before the treatment(<i>P</i><0.05). The variation coefficient of corneal endothelial cells in the observation group was more significant than that in the control group(<i>P</i><0.05).<p>CONCLUSION: Cataract patients with diabetes surgery tolerance is low, the corneal endothelial cell density and the central corneal thickness and corneal appears before tear secretion with stability and foundation treatment have significant changes, and its characteristic is cataract patients without diabetes more apparent, clinical intraoperative and postoperative corneal endothelial protection should be strengthened, and the surface of the eye protection ability of the organization.

@#AIM: To explore the effect of corneal limbal trapezoid conjunctival flap incision and modified Parks incision on tear film function and postoperative complications in patients with strabismus.<p>METHODS: A prospective study was conducted in patients with strabismus who had been treated with surgery in the First Affiliated Hospital of Nanyang Medical College between January 2017 and October 2019. The subjects were divided into conjunctival flap incision group(60 cases with 82 affected eyes treated with strabismus surgery <i>via</i> corneal limbal trapezoid conjunctival flap incision)and modified Parks group(62 cases with 83 affected eyes treated with strabismus surgery <i>via</i> modified Parks incision). Therapeutic effects on strabismus were evaluated, and dry eye symptoms were scored before and after surgery. Schirmer I test(SⅠt), tear film break up time(BUT)and cornea fluorescein staining score(FL score)were recorded. Comprehensive ocular surface examination was performed to determine tear river height(TMH)and noninvasive keratograph tear breakup time(NIKBUT). Satisfaction with surgery was surveyed, and adverse reactions were statistically analyzed.<p>RESULTS: Both groups of patients completed the surgery. Totally 80 eyes and 79 eyes of the conjunctival flap incision group and the modified Parks group were basically in normal position(within±10PD), and there was no statistically significant difference in cure rate between the two groups(<i>P</i>>0.05). S I t and BUT of the modified Parks group were significantly higher than those of the conjunctival flap incision group at 7 and 14d after surgery, and the FL score was significantly lower than that of the conjunctival flap incision group at 7d after surgery(<i>P</i><0.05). TMH of the modified Parks group was significantly higher than that of the conjunctival flap incision group at 14d after surgery, and NIKBUT was significantly longer than that of the conjunctival flap incision group at 7d and 14d after surgery(<i>P</i><0.05). The overall incidence rates of complications in the conjunctival flap incision group and the modified Parks group were 7.3% and 2.4%, respectively(<i>P</i>>0.05). The overall satisfaction rates of the conjunctival flap incision group and the modified Parks group were 83.3% and 95.2%, respectively(<i>P</i><0.05).<p>CONCLUSION:Surgeries with corneal limbal trapezoid conjunctival flap incision or modified Parks incision can achieve good correction effects on strabismus, but surgery with modified Parks incision has less effect on tear film stability, with higher surgical satisfaction and better clinical application effect.

ABSTRACT

@#AIM: To investigate the effects of fluorometholone combined with sodium hyaluronate eye drops in the treatment of xerophthalmia and the influence on inflammatory factors in tears. <p>METHODS: A prospective randomized controlled study was conducted in 116 patients(232 eyes)with xerophthalmia who were treated between February 2017 and December 2019. They were randomly divided into observation group and control group, 58 cases(116 eyes)in each group. The control group was treated with sodium hyaluronate eye drops, while the observation group was treated with 0.1% fluorometholone eye drops based on the treatment for the control group. Ocular Surface Disease Index(OSDI)scoring was carried out before treatment and after 2wk and 4wk of treatment. The tear film break-up time(BUT), Schirmer I test(SⅠt)and corneal fluorescent staining(FL)scores were measured. The tear meniscus height(TMH)and noninvasive keratograph tear breakup time(NIKBUT)were determined by eye surface comprehensive analyzer. Conjunctival impression cytology was performed to determine epithelial cell grading score and goblet cell density. Levels of interleukin 1β(IL-1β), interleukin -6(IL-6)and transforming growth factor β1(TGF-β1)in tears were determined. Meanwhile, efficacy and safety were evaluated.<p>RESULTS: The overall response rates of the observation group and the control group were 94.8% and 82.8%(<i>P</i><0.05). The observation group had higher SⅠt, BUT and NIKBUT, lower FL scores and OSDI scores than the control group at 4wk(<i>P</i><0.05). The goblet cell density was higher in the observation group than in the control group at 4wk(<i>P</i><0.05). IL-6 and IL-1β in the observation group were significantly lower than those in the control group at 2wk and 4wk, and TGF-β1 was significantly higher than that in the control group at 4wk(<i>P</i><0.05). The incidences of adverse reactions in the two groups were 3.4% and 1.7%(<i>P</i>>0.05).<p>CONCLUSION: Fluorometholone combined with sodium hyaluronate can significantly improve clinical symptoms as well as tear film stability in patients with xerophthalmia, which may be related to regulating effect on ocular surface inflammatory factors.

@#AIM: To analyze the therapeutic effect of loratadine combined with diclofenac sodium eye drops in the treatment of allergic conjunctivitis, and the influence on tear film stability and tear-related indexes.<p>METHODS: A prospective study was conducted among 93 patients(186 eyes)with allergic conjunctivitis admitted to the hospital between January 2019 and January 2020. They were randomly divided into observation group(<i>n</i>=47, 94 eyes, treated with loratadine and diclofenac sodium eye drops)and control group(<i>n</i>=46, 48 eyes, treated with loratadine). All patients received 2wk of treatment. The improvement of ocular symptoms and signs after treatment was evaluated. Schirmer I test(SⅠt)and break up time(BUT)were used to evaluate the tear film stability. The height, depth and cross-sectional area of lacrimal rivus were measured by anterior segment related optical coherence tomography. Tear specimens were collected to detect changes in tear hyaluronic acid(HA), group ⅡA secretory phospholipase A2(sPLA2-Ⅱa and eosinophil cationic protein(ECP). The occurrence of adverse reactions was counted.<p>RESULTS: After 2wk of treatment, the scores of main symptoms and signs were reduced in the two groups(<i>P</i><0.05), and the observation group had lower scores than the control group(<i>P</i><0.05). SⅠt and BUT were increased in the two groups(<i>P</i><0.05), which were longer in the observation group than in the control group(<i>P</i><0.05). The height of lacrimal rivus increased, depth increased, and cross-sectional area were increased in the two groups(<i>P</i><0.05). Besides, the above indexes in observation group were higher than those in the control group(<i>P</i><0.05). Meanwhile, HA, ECP and sPLA2-Ⅱa were decreased in the two groups(<i>P</i><0.05), which were lower in the observation group than in the control group(<i>P</i><0.05). No significant differences were found between the 2 groups in the incidence of adverse reactions(12.8% <i>vs</i> 10.9%, <i>P</i>>0.05).<p>CONCLUSION: The overall effect of loratadine combined with diclofenac sodium eye drops is better than that of loratadine alone in the treatment of allergic conjunctivitis. The combined treatment can improve symptoms, signs and tear film stability, reduce inflammatory mediators in tears, and promote recovery of tear film function. Besides, it is safe and feasible.

ABSTRACT

PURPOSE: To evaluate clinical findings in phlyctenular keratoconjunctivitis patients and assess the function and morphology of Meibomian glands using an interferometer (LipiView®, TearScience, Morrisville, NC, USA) in such patients.METHODS: This retrospective study included 19 eyes of 13 patients diagnosed with phlyctenular keratoconjunctivitis. The lipid layer thickness (LLT) and meibograph of each eye was quantified by tear interferometry. Tear film break-up time (TBUT) and corneal staining score were measured. Meibomian gland morphology (lid margin vascularity, plugging of gland orifices, lid margin irregularity, lid margin thickening, and partial glands) was evaluated based on anterior photographs and meibographs.RESULTS: The mean age was 21.3 years (8–44 years). Mean BUT and Oxford corneal staining scores were 2.6 ± 1.2 seconds and 1.9 ± 0.8, respectively. Abnormal findings of the Meibomian glands were observed in all patients. The mean LLT was 79.6 ± 27.4 µm and the incomplete eye blinking frequency was 3.8 ± 5.9 during 20 seconds. The graphs of the tear lipid layer showed various patterns such as flat, up-hill, down-hill, and mixed.CONCLUSIONS: Meibomian gland dysfunctions and changes in the tear film lipid layer were noted in patients with phlyctenular keratoconjunctivitis. These factors are to be considered for the treatment of phlyctenular keratoconjunctivitis in young patients under 10 years of age.

@#AIM: To investigate the clinical analysis of dry eye and tear film stability in pterygium patients after autologous conjunctival flap transplantation. <p>METHODS: Totally 114 cases of 138 eyes with pterygium were selected and divided into 56 groups of 67 eyes in the traditional group and 58 eyes of 71 eyes in the experimental group. The SⅠt values and BUT values of the two groups were observed before, 1 and 3mo after the operation, FL value, eye surface temperature and dry eye score of the two groups after surgery.<p>RESULTS: The total effective rate of the traditional group was 83.6%, and the total effective rate of the experimental group was 95.8%. The difference between the two groups was statistically significant(χ2=5.207, <i>P</i>=0.022); there was no difference in the SⅠt value before surgery between the two groups(<i>P</i>>0.05 ), SⅠt values in the experimental group increased significantly at 1, 3mo after surgery compared with the traditional group(both <i>P</i><0.05); there was no difference in BUT values before surgery in the traditional and experimental groups(<i>P</i>>0.05), and at 1 and 3mo after surgery the BUT value in the group was significantly increased compared with the traditional group(both <i>P</i><0.05); there was no difference in the FL score between the traditional group and the experimental group before surgery(<i>P</i>>0.05).FL value was decreased in the experimental group campared to the traditional group 1, 3mo after surgery(All <i>P</i><0.05); there was no difference in eye surface temperature between the traditional group and the experimental group(<i>P</i>>0.05), and the dry eye score test group was significantly lower than the traditional group(<i>P</i><0.05).<p>CONCLUSION: Autologous conjunctival flap transplantation is superior to simple tendon resection, which can promote the recovery of tear film stability in patients with pterygium, and the dry eye symptoms of patients are significantly improved.

OBJECTIVE@#To compare the therapeutic effect between acupuncture and hyaluronic acid sodium eye drops for dry eye syndrome.@*METHODS@#A total of 60 patients with dry eye were randomized into an acupuncture group and a western medication group, 30 cases in each one. In the acupuncture group, acupoints around the eyes, abdomen and limbs were selected, such as Jingming (BL 1), Cuanzhu (BL 2), Fengchi (GB 20), Taiyang (EX-HN 5), Guanyuan (CV 4), Qihai (CV 6) and Hegu (LI 4), once 2 days for 1 month, 15 times in total. In the western medication group, hyaluronic acid sodium eye drops was applied, 3 times a day, one drop every time, for 1 month. The tear meniscus height (TMH), tear break-up time (BUT) and conjunctival congestion scores were observed before and after treatment in the two groups, and the clinical effects of the two groups were compared.@*RESULTS@#The total effective rate in the acupuncture group was 68.3% (41/60), which was superior to 30.0% (18/60) in the western medication group (<0.05). Compared before treatment, the TMH was increased, BUT was prolonged and conjunctival congestion score was reduced after treatment in the acupuncture group (<0.05, <0.01). After treatment, the TMH and BUT in the acupuncture group were higher (<0.05, <0.01), and the conjunctival congestion score was lower than those in the western medication group (<0.05).@*CONCLUSION@#The therapeutic effect of acupuncture is superior to hyaluronic acid sodium eye drops. Acupuncture can promote lacrimal gland secretion and increase the tear film stability.

ABSTRACT

PURPOSE: To evaluate the efficacy of 3% diquafosol tetrasodium (DQ) after laser-assisted in-situ keratomileusis (LASIK). METHODS: This prospective randomized study included 150 eyes in 75 patients who were scheduled for LASIK. The patients in the 3% diquafosol tetrasodium (DQ) group (37 patients, 74 eyes) were instructed to apply one drop of DQ, six times daily post-op, while the patients in the 0.3% sodium hyaluronate (HA) group (38 patients, 76 eyes) were instructed to apply one drop of HA, six times daily post-op. A Schirmer test, tear film break-up time (BUT), corneal and conjunctival fluorescein staining score (FLSS), and ocular surface disease index (OSDI) were evaluated pre-op and at 1, 4, and 12 weeks post-op while the tear osmolarity was evaluated pre-op and at 4 and 12 weeks post-op. RESULTS: There was no significant difference between the two groups regarding Schirmer test results or tear osmolarity and conjunctival FLSS. The BUT was significantly higher in the DQ group at 1 week and 12 weeks post-op. The corneal FLSS was significantly lower in the DQ group at 1 week, 4 weeks and 12 weeks post-op. The OSDI was significantly lower in the DQ group at 1 week post-op. CONCLUSIONS: Use of 3% diquafosol tetrasodium after surgery improved ocular dryness and increased the tear film stability.

@#AIM:To observe the effects of recombinant human epidermal growth factor(rhEGF)eye drops on xerophthalmia after cataract surgery and its influence on tear film stability.<p>METHODS: The clinical data of 114 patients(158 eyes)with xerophthalmia after cataract surgery were retrospectively analyzed. According to the treatment methods of xerophthalmia, the patients were divided into study group(rhEGF+sodium hyaluronate group)and control group(sodium hyaluronate group). After 4wk of treatment, the efficacy and dry eye symptoms(dry eye questionnaire)were evaluated, and the BUT, SⅠt and FL were examined. The levels of IL-1β, IL-6 and TNF-α in tear were detected.<p>RESULTS: The total effective rate in study group was greater than that in control group(<i>P</i><0.05). After 4wk treatment, the BUT and SⅠt in the two groups were higher than those before treatment, and the indexes in study group were higher than those in control group(<i>P</i><0.01). The dry eye questionnaire score and FL and levels of tears IL-1β, IL-6 and TNF-α were lower than those before treatment, and the indexes in study group were lower than those in control group(<i>P</i><0.05).<p>CONCLUSION: Addition of rhEGF eye drops for xerophthalmia after cataract surgery can significantly improve dry eye symptoms, tear film stability, tear secretion and corneal epithelial cell integrity, and reduce inflammatory response, and it has significant efficacy.

ABSTRACT

@#AIM: To observe the effect of ultrasonic atomization of Shufeng Mingmu Decoction on tear film stability and index of tear fluid in patients with allergic conjunctivitis.<p>METHODS: Totally 140 patients with allergic conjunctivitis were randomly divided into 2 groups. 70 patients in the control group were treated with olopatadine hydrochloride eye drops combined with normal saline atomization therapy, while the 70 patients in the observation group were treated with ultrasonic atomization of Shufeng Mingmu Decoction combined with oldpatadine hydrochloride eye drops. After 14d of continuous treatment, the single symptoms(lacrimation, itching, photophobia, foreign body sensation), the stability of tear film(BUT, FL), and the index of tear fluid(ECP, HA, IgE)of the two groups were compared and analyzed. The clinical effects of the two groups were compared. <p>RESULTS: The total effective rate of the observation group was significantly higher than that of the control group(90.7% <i>vs </i>74.3%, <i>P</i><0.05). After treatment, the scores of single symptoms in both groups were lower than those before treatment, and those in the observation group were lower than those in the control group(<i>P</i><0.05). After treatment, the index of BUT in both groups were significantly higher than before treatment(<i>P</i><0.05), while the index of FL was significantly lower than before treatment, and the improvement degree of which in the observation group was better than that of control group(<i>P</i><0.05). After treatment, the levels of HA, ECP and IgE in the two groups were significantly lower than before treatment, and the improvement degree of which in the observation group was better than that in the control group(<i>P</i><0.05).<p>CONCLUSION: Ultrasonic atomization treatment of Shufeng Mingmu Decoction can improve the index of tear fluid of allergic conjunctivitis patients and the stability of tear film, which help relieve local allergy and improve clinical symptoms of patients.

ABSTRACT

In the evaluation of tear film stability based on corneal topography, a pretreatment algorithm for tear film video was proposed for eye movement, eyelash reflection and background interference. First, Sobel operator was used to detect the blur image. Next, the target image with highlighted ring pattern was obtained by the morphological open operation performed on the grayscale image. Then the ring pattern frequency of the target image was extracted through the Hough circle detection and fast Fourier transform, and a band-pass filter was applied to the target image according to the ring pattern frequency. Finally, binarization and morphological closed operation were used for the localization of the ring pattern. Ten tear film videos were randomly selected from the database and processed frame by frame through the above algorithm. The experimental results showed that the proposed algorithm was effective in removing the invalid images in the video sequence and positioning the ring pattern, which laid a foundation for the subsequent evaluation of tear film stability.

OBJECTIVE@#To analyze the effects of intradermal needling for pain and tear film stability in patients after pterygium excision.@*METHODS@#A total of 76 patients (98 affected eyes) with primary pterygium were randomly divided into an observation group (38 cases, 53 affected eyes) and a control group (38 cases, 45 affected eyes).In the control group, only pterygium resection was performed, in the observation group, intradermal needling after pterygium resection was applied at Cuanzhu (BL 2), Yuyao (EX-HN 4), Taiyang (EX-HN 5), Sibai (ST 2), Hegu (LI 4), removed after 24 h and changed three times a week. The pain level of 3 days after surgery, dry eye symptoms, the basic tear secretion test (Schirmer-Ⅰ), and the tear-break time (BUT) changes before surgery, 2 weeks after surgery and 4 weeks after surgery were compared between the two groups, and the clinical efficacy was evaluated.@*RESULTS@#The pain level of 3 days after surgery in the observation group was significantly lower than that in the control group (<0.05). The dry eye symptom scores at 2 weeks and 4 weeks after surgery in the two groups were significantly lower than those before surgery (all <0.05), and the dry eye symptom scores in the observation group were significantly lower than those in the control group (both <0.05). The Schirmer-Ⅰ test at 2 weeks and 4 weeks after surgery was significantly prolonged than that before surgery(all <0.05), and the Schirmer-Ⅰ test in the observation group was significantly longer than that in the control group (both <0.05). The BUT at 2 weeks and 4 weeks after surgery in the two groups was significantly longer than that before surgery (all <0.05), and the BUT in the observation group was significantly longer than that in the control group (both <0.05). The total effective rate in the observation group was 89.5% (34/38), which was higher than 71.1% (27/38) in the control group (<0.05).@*CONCLUSION@#Intradermal needling can effectively reduce the pain level of patients after pterygium resection, improve dry eye symptoms, promote the secretion of tears and improve the tear film stability.

· AIM:To investigate the effect of sodium hyaluronate eye drops combined with vitamin A palmitate ophthalmic gel on levels of tear film stability and inflammatory cytokines in patients with dry eye.· METHODS:A total of 100 patients with dry eye treated in our hospital from January 2015 to February 2017 were randomly divided into control group and observation group,50 cases in each group.Patients in the control group were treated with sodium hyaluronate eye drops.Patients in the observation group were given vitamin A palmitate ophthalmic gel on the basis of the control group,and then the clinical efficacy,tear film stability and the level of inflammatory cytokines were detected in the two groups.· RESULTS:After treatment,the levels of BUT and S I t in both groups increased significantly compared with that before treatment,and FL was significantly lower than that before treatment.The levels of BUT and S I t in the observation group after treatment were 11.24±0.22s and I1.4 ± 0.17mm/5min respectively,which was high than that of control groups(P<0.05).FL level was 1.78±0.10 points,which was significantly lower than that of the control group after treatment (P<0.05).After treatment,the levels of IL-1β and TNF-αsignificantly decreased in both groups.The levels of IL-1β and TNF-α in the observation group were 34.38± 5.58ng/L and 134.47 ± 12.14ng/L,significantly lower than those of the control group after treatment,the difference was statistically significant (P < 0.05).The total effective rate of observation group after treatment was 91.7%,significantly higher than that of the control group after treatment,the difference was statistically significant (P< 0.05).· CONCLUSION:Sodium hyaluronate eye drops combined with vitamin A palmitate ophthalmic gel can relieve the symptoms of patients with dry eye effectively,increase the stability of tear film,and reduce the levels of inflammatory factors in tears,which is reliable in clinical application.

ABSTRACT

AIM:To investigate the effect of recombinant bovine basic fibroblast growth factor (rb-bFGF) eye drops and hydroxyl indican eye drops on tear film stability and dry eye symptoms after age-related cataract surgery.METHODS:Totally 80 patients with 80 affected eyes undergoing age-related cataract surgery in our hospital from January 2015 to October 2016 were selected as study subjects,and they were randomly divided into control group and experimental group with 40 patients (40 affected eyes) in each group.The two groups were treated with hydroxyl indican eye drops and rb-bFGF eye drops,respectively.The clinical curative effect,inflammation related factors [interleukin 6 (IL-6),tumor necrosis factor α (TNF-α)],Schirmer test (S Ⅰ t),break-up time (BUT) of tear film,corneal sodium fluorescein staining (FL) and scores of dry eye symptoms in the two groups were observed.RESULTS:The total treatment effective rate of experimental group after treatment was significantly higher than that of the control group (90.0% vs 72.5%;x2 =4.021,P< 0.05).Before treatment,there was no significant difference in IL-6,TNF-α,S Ⅰ t,BUT,FL score and scores of dry eye symptoms between the two groups (P>0.05).After treatment,IL-6,TNF-α,S Ⅰ t,FL score and scores of dry eye symptoms in two groups significantly decreased while BUT significantly increased (P< 0.05),and changes of the indexes were more significant in the experimental group than the control group (P<0.05).CONCLUSION:In the treatment of patients after age-related cataract after surgery,rb-bFGF eye drops has more advantages over hydroxyl indican eye drops in regulating the expression of inflammatory factors,improving the tear film stability and relieving dry eye symptoms.

ABSTRACT

Objective To study influence of travoprost eye drops in treatment of after anti-glaucoma surgery and its effects on visual function, the tear film stability and inflammatory index. Methods 90 patients of anti glaucoma who received therapy from August 2014 to August 2015 in the first hospital of Ninghai County Zhejiang Province were selected as research objects. The control group was treated with timolol maleate, while the observation group was treated with travoprost eye drops, then the visual acuity, intraocular pressure, tear film stability (rupture time, schirmer), inflammation (flashlight, cell count), visual field defect percentage range, curative effect after treatment were compared. Results After treatment, the vision in observation group (0.95±0.26)D was better than the control group (0.76±0.21)D, intraocular pressure in observation group (11.29±3.23) mmHg was less than the control group (13.89±3.72)mmHg, the difference was statistically significant (P<0.05), rupture time in observation group (12.93±1.90)s was higher than the control group (10.36±1.80)s, schirmer in observation group (13.01±1.60)mm was higher than the control group (11.10±1.02)mm, the difference was statistically significant (P<0.05), TNF-α, IL-6 in observation group was less than the control group, the difference was statistically significant (P<0.05), field of visual field defect in observation group (38.96±10.21)% was less than the control group (47.37±11.35)%, the difference was statistically significant (P<0.05), the total effective rate of observation group 95.56%(43/45) was statistically higher than that in the control group 77.78%(35/45), the difference was statistically significant (P<0.05). Conclusion Travoprost eye drops can improve glaucoma postoperative visual function, tear film stability, reduce inflammation.

@#AIM: To analyze the application value of corneal limbus stem cells transplantation in the treatment of pterygium. <p>METHODS: Totally 300 patients(318 eyes)with pterygium were divided into 2 groups according to treatment methods. Patients of the control group(70 patients with 77 eyes)were given simple surgical resection, while patients of the observation group(230 patients with 241 eyes)were given corneal limbus stem cells transplantation after surgical resection. The postoperative recurrent rate, corneal epithelium incision healing time, corneal epithelium healing degree, pain score and tear film break-up time were observed. <p>RESULTS: The corneal epithelium incision healing time of the observation group and control group were respectively 5.2±1.9d and 6.4±1.7d, and the difference had statistical significance(<i>P</i><0.05). There was no patient recurred in the observation group, while there were 7 eyes(9.1%)recurred in the control group, and the difference between groups had statistical significance(<i>P</i><0.05). At 1wk after treatment, there was no significant difference in corneal epithelium healing degree and pain score between groups(<i>P</i>>0.05). At 2wk after treatment, the corneal epithelium healing degree of the observation group was better, and the pain score of the observation group was lower than those of the control group, the differences between groups had statistical significance(<i>P</i><0.05). Before treatment, there was no significant difference in tear film break-up time between groups(<i>P</i>>0.05). At 1 and 2wk after treatment, the tear film break-up time of the observation group was longer than that of the control group(<i>P</i><0.05). <p>CONCLUSION: Corneal limbus stem cell transplantation can prevent postoperative recurrence of pterygium effectively, and its postoperative tissue repair effect is superior to simple surgery.

ABSTRACT

AIM:To compare and analyze the effect of recombinant bovine basic fibroblast growth factor ( bFGF ) eye drops and hydroxyl indican eye drops on the tear film stability and dry eye symptoms after age-related cataract surgery. ·METHODS: A total of 115 patients ( 115 affected eyes ) with dry eyes after age-related cataract surgery were divided into the bFGF group, the hydroxyl indican group and the blank control group by the random number table method. The blank control group was only given routine anti-inflammatory treatment, and on the basis, the bFGF group and the hydroxyl indican group were treated with bFGF eye drops and hydroxyl indican eye drops respectively. The clinical efficacy, adverse reactions, changes in scores of dry eye symptoms, Schirmer test ( SⅠt) , tear film break- up time ( BUT ) and corneal fluorescein staining ( CFS) scores at different time points were compared among the three groups. ·RESULTS:The total markedly effective rates in the bFGF group ( 89. 5%) was higher than that in the hydroxyl indican group ( 70. 3%) or the blank control group (47. 5%) (P<0. 05). Scores of dry eye symptoms, CFS, SⅠt and BUT in the three groups at 2 and 5wk after surgery were significantly different (P<0. 05). The above-mentioned indexes in the bFGF group at 2 and 5wk after surgery were significantly better than those in the hydroxyl indican group and the blank control group, and better in the hydroxyl indican group than the blank control group ( P < 0. 05 ). There were no obvious adverse reactions. ·CONCLUSION: The tear film is unstable in early stage after age-related cataract surgery, and there are dry eye symptoms. The intervention with bFGF eye drops and hydroxyl indican eye drops can effectively restore the tear film stability and significantly relieve dry eye symptoms, and the effect of bFGF eye drops is more significant.

ABSTRACT

Objective To observe the changes of tear film stability,goblet cell and mucin 5AC expression in conjunctivochalasis patients,and explore the mechanism of conjunctivochalasis.Methods Conjunctivochalasis patients (30 cases) and single age-related cataract patients (15 cases) were collected as conjunctivochalasis group and normal control group.Eye symptom assessment (OSDI score),tear break-up time (BUT),Schirmer Ⅰ test,tear fern crystallization tests were performed for all the selected persons.Conjunctival rescent-shaped resections were made for all the conjunctivochalasis patients.Conjunctival tissue samples were stained by HE staining,AB staining,mucin 5AC immunohistochemical staining from the conjunctivochalasis group and norral control group respectively,and then statistical analysis was made.Results The OSDI score in the conjunctivochalasis group (37.80 ± 8.94) was significantly higher than that in the normal control group (11.40 ±4.08) (P <0.01).BUT in the conjunctivochalasis group (6.70 ± 2.76) s was significantly lower than that in the normal control group (13.67 ± 3.48) s (P < 0.01).Schirmer Ⅰ test in the conjunctivochalasis group (6.23 ± 3.13) mum was significantly lower than the normal control group (13.40 ± 3.74)mm (P < 0.01).Tear ferbing crystallization of the conjunctivochalasis group was decreased significantly compared with the normal control group (x2 =14.309,P =0.003).Light microscopic showed that conjunctival thickness was thinned,collagen fibers were less,elastic fiber was reduced,the lamina propria and interstitial were congestion and edema,the number of goblet cells was significantly reduced,and the positive staining of mucin 5AC staining was significantly lower in the conjunctivochalasis group than in normal control group (x2 =9.499,P =0.023).Conclusion For patients with conjunctivochalasis,the tear film function is affected,goblet cells are decreased,tear fern crystallization is decreased,mucin 5AC content is decreased,which finally leads the excessive conjunctival relaxation and abnormal ocular surface and tear.

ABSTRACT

AIM: To investigate the effects of different phacoemulsification incision for tear film stability in cataract patients. METHODS: From January 2014 to September 2015, 108 patients (144 eyes) who underwent cataract surgery were divided into two groups: group A with clear corneal incision ( 52 cases with 74 eyes), group B with scleral tunnel incision (56 cases with 70 eye). Two groups of patients were taken the same incision width. Symptoms of dry eye and tear film break - up time ( BUT) were observed 1d before and 1d, 1, 3wk, 1 and 3mo postoperatively. RESULTS:After performing cataract phacoemulsification 1d, 1wk, symptoms score of dry eye of group A was less than that of group B, the difference was statistically significant (t = -6. 199, -2. 871, P0. 05); at postoperative 1d, 1 and 3wk, BUT of group B was better than those of group A, there were a statistically significant differences ( t = - 3. 718, -2. 342,-2. 506, P0. 05).CONCLUSION: The difference on effect of two different incision on the tear film stability only last a short time, which disappeared at 1mo after operation. The clear corneal incision has stronger impacts on tear film stability than scleral tunnel incision, while with a lower score on symptoms of dry eye.
